Have you ever experienced an overdraft when making a purchase with your debit card or withdrawing money at the ATM?

> Yes               > No

When making a purchase or an ATM withdraw with your debit card when not enough funds are available in your account, would you like us to authorize the transaction?

> Yes               > No

If you answered no to the last question, would your opinion change if the transaction happened to be for an important purchase such as groceries or gasoline?
> Yes               > No

If you answered no to the last question, would your opinion change if the transaction happened to be for an emergency purchase such as a medical expense or car tow?

> Yes               > No

If you answered yes to any of these questions you should Opt-In to the Courtesy Pay Program.

If you answered no to all of the questions above, you might not need to Opt-In. However, keep in mind if you do not have enough money in your account to cover a transaction that transaction will be declined.
